Hardcoding some cases like "ignore north for mushroom_stem if there will be a block north of it" would also be a simple short term solution, but I don't like it...

It's just further narrowing down the cases in which the property is ignored and having the behavior hardcoded rather than as a default configuration value.

That particular behavior is new but there is a known and fixed bug involving negative y coordinates in `FasterWorldScanner` so you should try updating Baritone.
