insertHtml Prepends Content

I noticed that when I try to `insertHtml('');` to a `Medium.js` instance after calling `medium.focus()` off a button click, the content is always prepended. Not sure what's exactly going on,...

Automate releases with Travis

In the README, `lockShared` and `unlockShared` are documented but aren't exposed on the mutex object returned from the mutex creator factory.

HTTP streaming would be dope. If you don't have a lot of time, I'm fine with looking into it and seeing if I can implement this (forked the repo).

The current code comment is `8.5MB`, but it should be `85MB`: Just doing god's work
