I understand WSL is the way to go for using Nutils in a Windows environment (I wasn't aware.) Does it make sense to elaborate on this in the readme?
For bivariate elements, the importer asserts that all coordinates are in the x-y plane, but we would like to be able to import curved surfaces too.

Extend the merged #83 to mixed bases. Changes are more extensive than anticipated: I'll probably have to work in ASMmxBase::establishBases() too.
