It is grabbing the photo from the source. Perhaps you have the workshop version installed as well?
What dkoz said is correct, you must specify the requirements.
You need to rename the folder, as addons with a - in their name will not work. Rename it to "levels" or something.
Do you perhaps have the github version on the server, but also the workshop version added to the resources?
