In my experience the buttons do appear ... on the main screen, which is not entirely obvious at first.
Importantly, if you click on that OTP field after pressing enter - it will be automatically converted to `otpauth://` URL and the record will be marked as modified.

@TonyRL I found the reason of LIMIT_INVALID errors and fixed it. Also pushed html unparse() fixes to get proper formatting.
