We also ended up with 2 accounts for the same user, apparently due to user double clicking the signup button. Manually added unique: true to our user field.

Oh, somehow it seems audience array is an hidden feature of auth0/node-jsonwebtoken: https://github.com/auth0/node-jsonwebtoken/blob/v5.1.0/index.js#L191 It be great to update the multi-tenancy README example to mention this.
I'm also running in a similar issue. Depending of situation heartbeat does sometime seems to kick in ie. I did get "no heartbeat or data in last xx seconds" exception...

I think obj_name is leaked here. Who's responsible for freeing obj_name? Could probably NanBufferUse to hand off the buffer to V8.
