14.04 LTS is not anymore in maintenance updates and now only in extended security maintenance (https://ubuntu.com/about/release-cycle), so I suggest to skip wasting time to support 14.04.

There is no "http://hr-city.survey.okfn.org" *.survey.okfn.org like you suggested. So there is still existing problem with *.index.okfn.org site. Any plans to fix these issue or migrate index sites to survey?
Maybe better instead of created_at to use updated_at? Since we have examples in Croatia where requests get response after few months and more (sometimes even 1y after request gets sent)....
