Support variables in settings.json

Certain settings like `clang-tidy.buildPath` should support variables like `${workspaceFolder}`.

It looks like clang-tidy's yaml output contains the severity of diagnostics now. We should use that instead of grabbing the severities from the human-readable output when possible.

When you change the extension's settings, all open files are re-linted. #16 ensures that only one instance of clang-tidy ever runs at a time. This breaks the re-linting process. If...
