Hi Apparently I ran into the same issue, although I saw `msg="error removing stale clients" err="too many failed ingesters"` as well in the logs. Having a look at the code...

@bboreham what was the idea behind not removing ingesters from the ring when they are suspected to be dead? and then what is the purpose of `removeStaleClients` and `cleanUnhealhty` ?...
