I also encountered this problem when using rtl_macro_placer command (inside macro_place_util.tcl), my workaround is to use place_pins before calling rtl_macro_placer. I'm not sure if it is strictly required to place...
@val-terry I added this command to the first line of macro_place_util.tcl: place_pins -ver_layers metal4 -hor_layers metal3.
