@geerlingguy here is mine test with the image you mentioned above  Using putty on Windows 11 with FT232B USB to Serial dongle. GPIO14 as TX and GPIO15 as RX...
I don't think there is any issue with the design as there are many guy use my design and sell it.
May I get more info, what game and what emulator you are trying?
I would ask you to copy all settings of retroarch cfg and try again. As I just do everything again on a new SD card, it works for me.
I think you must missed some part of tutorial. plz make sure you both modify cmdline.txt and also config.txt.
