I've updated the code to allow you to rename the uncategorized tag, but the Package Hierarchy isn't updating to display the tag.

It appears that the fix from Zinc was never included in Pharo 11-13.
Can you post the screenshot of the whole view? I can't see if you have a filter that is hiding some of the items.
Did you make some changes in the source view and then switch to the grammar view?
