Update to both incorporate the interface changes in gdcm-rs and dicom-rs.
with the relese of gdcm-rs 0.4.0 this should hopefully build now
hot damn, some more support for encoded pixel data ready to go!

@AndydeCleyre if you can lock the dev depencencies specified in `tests_require` it can be resolved.
