Edit form entry doesn't edit all fields

When a form is clicked on to edit or duplicate only the fields the user filled out prior are available and not all pages of the form.

Have a sales rep in our organization that has a Galaxy Note 5 and the app just crashes. I have attached the screenshot and we have clicked the "report" button...
