Exactly like the solution in #18 but I went ahead and made change styling changes suggested by @phongsi and also updated the example applications to show the default OmniAuth failure...

I made the last change to the main branch a year ago and I don't have the ability to publish a release. I'm not seeing any other maintainers jump in...
