Did you figure out why? I am using Crispy forms as well, and this happens to me (but it does not happen in the admin). @sureshvv
It seems that with crispy forms you do not need to include the {{form.media}} in the header, as crispy includes this info in the form content. I removed the {{form.media}}...
Any update on this @WrathOfZombies ? How do you get the id_token attribute? It is still not handled...
