- add support for 8-bit characters (\'xx) - fix issue with generating empty HTML files in some cases - remove possibly redundant std::logic_error which causes an error on some files

Comments are not included into hash calculation

I did some testing and it looks like that adding `NOLINT*` comments does not affect hash calculation. As a result `ctcache` might report source file as "good" after `NOLINT` comment...
