`node-fetch` does not work behind a proxy

Just hit the same problem using [live-plugin-manager](https://github.com/davideicardi/live-plugin-manager/issues/48). I also agree that this should be added to node-fetch instead of every project using node-fetch having to implement their own proxy handler.
