`code:which(Module)` return archive file path (for example: `"/app-1.0.ez/app-1.0/ebin/my_mod.beam"`). I think that the module should be loaded into the memory if failure to write by any reason.

You should stop [lager_middleman_server](https://github.com/klarna/lager_middleman_backend/blob/master/src/lager_middleman_server.erl#L92). `link` not working with gen_event handlers and gen_server will never receive kill message.
