I do have the same issue with TachiyomiSY (latest version based on 3bc6b1e202f8344356720467c60da19239e22ebc) and they appear after keeping the app open for like 30+ minutes. It just became very slow...

IMHO a primary key out of FK1 and FK2 would be best. Like users_groups: PRIMARY(user_id, group_id) At the moment you can add the same entry multiple times which does not...
