Add the FSH files for the API examples

This pull request adds the FSH files needed to generate the FHIR examples needed for the examples in the API repository

The API-Examples folder contain files have known FHIR validation errors. This is a list of those along with the reason that they do.

Update the erp_eml-epa-notes examples
