Improve documentation for best practices around specializing fmt::formatter

It's not clear from documentation should I use FMT_STRING or simple string literal in case of I need the best performance?

result::is_null returns incorrect value for unbound columns

It look like ODBC's architectural feature/flaw when you can stream unbound amount of data but stream size/status will be available only after stream is fully read.
