Fix APIM 4.2.0 DB2 SQL files

Description

The db2.sql script contains several issues which result in syntax errors. We need to port applicable fixes from https://github.com/wso2/api-manager/issues/2762

Steps to Reproduce

  1. Configure latest 4.2.0 with db2
  2. Attempt to create a new revision and deploy and API
