"User denied media access"

This has started to happen sporadically. Only on Windows. Several user reports.

I have confirmed:

1. User has allowed camera and microphone access for the site
2. User has allowed Chrome (and other apps) to access camera and microphone in Windows privacy settings

Restarting the computer seems to fix it.

Any guidance on how to debug this further?

fwiw I've seen the same with some of my users, but I believe the issue is Chrome or Windows, as simply refreshing the page or even restarting Chrome doesn't seem fix it.
