No inbound/outbound Calls: 11200 HTTP | HTTP 500

I set up a new installation of OpenVbx. when I call the number I receive a an "unknown error" message.

Upon checking Twilio, it shows a 11200 HTTP retrieval failure, with an HTTP status code of 500

With this setup, I am unable to make or receive calls.

I am using Arvixe shared hosting for this installation.

Any guidance would be appreciated for what is going wrong and how to fix it.

Do you have the Application SID filled out in Settings->Twilio Account?

Also, can you post the request info that Twilio used to call your install of OpenVBX? You can get that information from the call log section of the Twilio Account Portal.
