how to insert item?

when i insert a item , it will crash , this is my log *** Terminating app due to uncaught exception 'NSInternalInconsistencyException', reason: 'no UICollectionViewLayoutAttributes instance for -layoutAttributesForItemAtIndexPath: <NSIndexPath: 0xc89c390> {length = 2, path = 0 - 5}'
