Consolidate change logs

Description of the infrastructure enhancement. Right now, change logs show up in 3 places. The upgrade notes and changelog in documentation and the CHANGELOG in tilemill. Let's get that down to no more than 2 changelogs. Also, the documentation/changelog (which appears to be generated from somewhere) is currently out of date.

Reason for improving this aspect of the infrastructure? Less to update when we do a release.
