Append or Prepend with markdown file when updating

Hi,

First of all thanks for putting this lib out, it's so handy!

Would be possible to append or prepend the Confluence page with the given MD file on the update process?

My suggestion is to add the flags --append or --prepend.

I can possibly also work on this in the future and create a PR, just not in the near future :)

What is the purpose of appending or prepending?

Could you just update the Markdown with the whole contents and publish again?
