Manual order in Bookmark

Right now, bookmark serves a nice functionality, but if manual ordering is supported, it will be more handy.

Order is set by the writer from TOC so you need to edit the source. Altering display into alphabetic or other order is counter intuitive that's why a document index is added as look-up appendix usually in alpha order
