Issue #204: remove modules and mentions of mod_mp4, mod_mp4v, and mod_mp4v2

Addresses Issue 204. Additional context in Issue 188.

Is an expected outcome of removing this module that .mp4 files are no longer generated? I removed all mentions of .mp4 files under this assumption. If that is not true, it may be that the line in mod_conference can be kept in.
