Renames the usages of the Cosmos CHAIN_NAMESPACE to be CosmosSdk following https://github.com/shapeshift/lib/pull/1008

⚠️ Merge as a stack from Graphite since this requires CAIP version bump and will break unless all CHAIN_NAMESPACE changes are updated at once
