Not workin while calling the modal with javascript

In my case is working when I use the hard coded data attributes. When I try to call it with javascript it is not working and in some cases it will show the modal not at the needed position. I was also trying to get wise with your fiddle example but this is not working either.

Because I have the need to add different form elements elements dinamicaly I reaaly have to use jquery, so any help with this will be appreciated. What I am doing wrong. Thank you
