Add page links to table of contents pages for PDF

Add page bookmarks to the PDF tablet of contents.

@computergeek1507 Thanks for the ticket. This is defo on the list of things we want to do. The only reason we didn't address this yet is due to a bug in the underlying PDF library we're using that prevented bookmarks from working.
