Ehm, no.

You probably didn't store it correctly. The tape also seems to show that you were tempering with it – proof me wrong :-)

But cool, the last one I've seen is my own last floppy of the TurboAss.

I can only guess that I taped that label on the back to remember an update? I am also sure I could find dozends of printed (NEC P2000) pages from support letters and answers.
