assign root-crl-url to root-ca.crt and crl-url to ca.crt

issue #17.

This PR aligns the behavior of the crl url params with how I was expecting it to work... which I think is what was originally intended?

ping. Revisiting open PRs.. This puts the root CA CRL URL into the root CA cert, and the intermediate CRL URL into the intermediate CA cert.
