What's in this Pull Request?

I split the adaptive card into a template JSON and a data JSON to make it easier to update the data. Let me know if this is OK - I'll work on the other cards as well.

I haven't updated the readme yet, but I will add that part when I get a response to if this is an OK approach or not.
