Is your feature request related to a problem? Please describe. No, I'm base in a necessity and would like to know if there is something that I can do or if is possible to add as feature.

Describe the solution you'd like Is it possible to add a followup in a simplified template (using Self Service Profile) with followup based in a template model, like the image:

Describe alternatives you've considered There is a possibility to add without change something?

Additional context When I use Self Service Profile I cant add a followup with an template model. Just I'm using a user that have another profigle.

Your question seems related to GLPI, not Formcreator, am I right ?

Hi @btry , I think is related with FormCreator, because I`m using the Self Service Profile with the Simplified template in the profile.
