Describe the bug

• Can't create a database from the "Mod Input Options" form

To Reproduce

Steps to reproduce the behavior:

1. Add a DB field with "Database created at activation"
2. Click over to "Mod Inputs Options" form
3. Click "+ Add new"
4. Try to create a personal database

Actual behavior

• Initially I could type in a DB name, but the Create Database button was disable
• Later whenever I typed, the modal would just disappear
