Bumped versions of NPM packages. Gitignoring files generated in 'dist'.

• Bumped the versions of all NPM dependencies.
• Replaced rollup-plugin-uglify with rollup-plugin-terser since the former isn't compatible with recent versions of rollup.
• Had to add preventAssignment: true in the rollup config files because some defaults changed in plugin-replace and I wanted to keep the build warning free.
• Deleted the generated js and map files from source control.
