Per page table of contents missing

The old manual has a table of contents per page at the top of the page. The new manual has no obvious per page table of contents visible for a given page.

For the record, this can be explicitly added for pages that need it using a .. contents:: directive. It looks like some pages had __TOC__ inserted in MediaWiki to explicitly request a ToC.
