Adds OLM flow diagram for troubleshooting

The OLM flow diagram helps understand how information flows into, through and out of OLM with the objective of improving how we figure out what area an action is failing at and how to determine that through deeper investigation.

I've included the SVG version in hopes that this can be the source and editable on a larger scheme. This file did open correctly in Chrome. I originally created it with Visio so that's not very open source friendly, but @awgreene was hoping to have it here so maybe we can find a way to get there. I'm willing to spend a bit of time reworking it in another format if required.
