Adapt FHIR resource providers to reflect published implementation guide

There is a published implementation guide for MHD and PDQm:

Specifically:

  • extended operation for PIXm appears twice
  • duplicated author parameters for DocumentManifest/Reference
  • search parameter descriptions?
