Replace 'fetch' with an npm registry follower.

That would allow to switch back to replicate.npmjs.com, which should include scoped packages. Also that would allow to build real-time updates without re-downloading the whole dataset.

Ok, looks like skimdb.npmjs.com has been shut down one-two days ago? We need to migrate to replicate (and sync) asap =).
