Add an index page of previously created Audiograms

with MongoDB and AdcomNYT index page conventions.

We should not expend more than a very small amount of effort here at this stage. (Until we prove usefulness of the tool.)

If we are storing this stuff, we can also at least record who created a given audiogram.
