Stops working after OBS is restarted

While Midi Mixer is active, restart OBS. The controls stops working. Midi mixer needs to be either restarted or the plugin manually reconnected.

I'm hoping to see this issue to be fixed in the next release, probably some kind of onDisconect event handling to reconnect every second.
