Broken Controls display

We are using a copy of this page https://r4fx.github.io/mediaelement-playlist/

and the controls shown inside the player have been broken very recently, it was showing ok before that. Assuming from a recent update, but I cant locate where the issue is exactly.

Can you please shed some light on what changes required, taking the above URL as reference ?
