Add option to create a new issue for smoke test faliures

We currently offer email and slack notifications. I think it would be prudent to create a new issue for smoke test failures (or append to an existing issue).

It would be good to check for an existing issue prior to creating a new one??? Just so we don't create many MANY issues automatically.
