Create and add an app icon

I created an app icon to help me find the running program easier since I had many Qt apps that used the default.

I got it working on both Windows and macOS but am new to CMake development so I'm not sure what the best practices are for icon formats, etc. Feel free to change anything but I hope this is helpful to the project.
