Keyboard shortcuts not working by default

Hi,

after a new installation of the extension, I had to remove and re-assign the keyboard shortcuts to make them work. Removing other shortcuts with the same keybind didn't help either.

Don't know if this is fixable from within the extension though, might be a VS Code thing. Just wanted to give a heads-up anyway.

I don't know how to make the keyboard shortcuts from this extension override the defaults. A PR to update documentation would be welcome if you had a known fix/keyboard shortcut
