[Module] Pids aren't resolved after the first start.

When a server reboot, the pid id change. PMX resolve the pid id when it start but not after. So when you restart a server that is tracked by pid, RAM/CPU usage are at 0.

Temporary fix : everytime to restart the server, restart the module that monitor it.
